Written pitch is what sax players read. Concert pitch is what sounds. E♭ saxes transpose differently than B♭ saxes, so the same written note can sound different across sax types.

Why do enharmonics change from G♯ to A♭ sometimes?

The tool adapts spelling to the chosen key to keep note names musical. You can override that with the note spelling control.
